Tesamorelin in 2026: a meta-analysis of the HIV lipodystrophy trials
Four randomized trials and 909 patients: visceral fat falls. That is not a general fat-loss or anti-aging result.
Tesamorelin is a stabilized analog of growth-hormone-releasing hormone. Unlike most “GH peptides” discussed online, it has FDA-approved products for a narrow indication: excess abdominal fat in adults with HIV-associated lipodystrophy.
The 2026 pooled analysis
A systematic review and meta-analysis searched through 9 February 2026 and included four RCTs (909 patients). Tesamorelin 2 mg reduced visceral adipose tissue, waist circumference and trunk fat, and increased lean mass. Lipid changes were modest. Growth-hormone-related adverse effects appeared, and discontinuation was more common than on placebo.
What the label already warned
Approved tesamorelin is not indicated for general weight-loss management. Long-term cardiovascular safety was not established in the original program. A 2026 meta-analysis of the same population does not expand the indication to bodybuilding or longevity.
Bottom line
Evidence ladder: Human RCT — for HIV lipodystrophy, not for unapproved uses. When people treat tesamorelin as interchangeable with sermorelin or CJC-1295, they are mixing a labeled drug class with research-chemical marketing.
